Ezetimibe plus a Statin after Acute Coronary Syndromes

چکیده
To the Editor: With regard to the article by Cannon et al. (June 18 issue),1 human serum cholesterol derives from two sources: it is either synthesized endogenously or absorbed in the intestine. Statins inhibit endogenous cholesterol synthesis, whereas ezetimibe inhibits intestinal cholesterol absorption.2 According to findings from the Scandinavian Simvastatin Survival Study3 and the German Diabetes and Dialysis Study,4 patients with a high level of cholesterol absorption may receive less benefit from statin therapy than those with a low level of cholesterol absorption. However, patients with elevated absorption may particularly benefit from the addition of ezetimibe to a statin regimen.2 Patients with elevated intestinal cholesterol absorption can be identified with genetic testing for risk alleles in the ATP-binding cassette transporters G5 and G8 (ABCG5/8), Niemann–Pick C1– like 1 (NPC1L1), and ABO genes.5,6 It remains to be investigated whether these alleles will predict whether statins will be less effective in reducing cardiovascular risk in patients harboring them than in those not harboring them. If so, testing for these alleles in addition to measuring lowdensity lipoprotein (LDL) cholesterol levels may be helpful in deciding when to add ezetimibe treatment to ongoing statin therapy. This would be an approach toward personalized prevention of cardiovascular disease.

متن کاملLow-density lipoprotein cholesterol targeting with pitavastatin + ezetimibe for patients with acute coronary syndrome and dyslipidaemia: the HIJ-PROPER study, a prospective, open-label, randomized trial
Aims To elucidate the effects of intensive LDL-C lowering treatment with a standard dose of statin and ezetimibe in patients with dyslipidaemia and high risk of coronary events, targeting LDL-C less than 70 mg/dL (1.8 mmol/L), compared with standard LDL-C lowering lipid monotherapy targeting less than 100 mg/dL (2.6 mmol/L). Methods and results The HIJ-PROPER study is a prospective, randomize...

permits unrestricted non-commercial use, distribution, and reproduction in any medium, provided the original work is properly cited. When platelets are activated, they increase in size and change from a quiescent disc to a swollen sphere. Large platelets are more reactive and likely aggregate more than small ones. 1 Mean platelet volume (MPV), an indicator of platelet size and activation , has ...
متن کاملStatin non-adherence: clinical consequences and proposed solutions.
Large controlled clinical trials have demonstrated reductions with statin therapy in cardiovascular events in patients presenting with acute coronary syndromes and stable coronary heart disease and individuals at high risk of a cardiovascular event. In trials of acute coronary syndromes and stable coronary heart disease, high-intensity statin therapy is more effective in the prevention of recur...
متن کاملTreatment with ezetimibe plus low-dose atorvastatin compared with higher-dose atorvastatin alone: is sufficient cholesterol-lowering enough to inhibit platelets?
OBJECTIVES We sought to test the platelet inhibitory and anti-inflammatory effects of a higher statin dosage compared with combined treatment with ezetimibe plus a low statin dose. BACKGROUND Reducing the level of low-density lipoprotein cholesterol (LDL-C) with statins induces important pleiotropic effects such as platelet inhibition. An insufficient LDL-C reduction often is treated with eze...
